The people who invented this
Born in our Harvard lab. ESG at its best.
Not all ESG is the same. Pattern, depth, the hands at the controls — every detail
decides whether you lose 20% or regain in a year. Everself ESG is the original
protocol, performed by the team that invented it.

"Not all ESG is the same. The pattern of sutures, the depth, the hands at the
controls — every detail compounds. We wrote the protocol so every Everself
patient gets the procedure done right."
A lifetime of GLP-1 drugs. Or a one-time procedure.
GLP-1s are a temporary boost to your satiety hormone, designed for
lifelong use. Most people who stop regain most of the weight back.
Research shows that switching from a GLP-1 to ESG leads to 18% more
weight loss after a year; switching to a stronger GLP-1 only gets
you 9% more.
You've tried GLP-1 — so what's next?
After ~10 months on GLP-1, weight change over the next year by next step.

I knew friends and family members who opted for the bariatric sleeve surgery. They were successful, but I wasn't sure if it was right for me. I made the decision to move forward with the non-surgical solution. 42lbs lighter — and I'm not done yet.
LisaVerified patient · June 2024
I was seeking a more permanent solution but was afraid of the conventional gastric sleeve. That's when I discovered the ESG procedure — less risk of infection, better absorption of vitamins, no scars. Three months later, I've lost around 30 pounds.
PatriciaVerified patient · May 2024
I started to research different bariatric options. I could not have chosen a better place than Everself. The procedure was easy and painless and I returned to work in 3 days. 5 weeks in, I've lost 30 lbs.

What is ESG stomach tightening (endoscopic sleeve gastroplasty)?
ESG stomach tightening — clinically called endoscopic sleeve gastroplasty (ESG) — is a 60-minute non-surgical weight-loss procedure. Your physician places a series of internal sutures through the mouth using an endoscope, folding and tightening the stomach into a banana-shaped sleeve without removing any tissue. No incisions, no scars, no hospital stay. The result is a smaller stomach that fills earlier during meals, paired with Everself's 12-month support program that builds the eating patterns to hold the weight loss.
What's the difference between ESG and VSG (vertical sleeve gastrectomy)?
ESG (endoscopic sleeve gastroplasty) and VSG (vertical sleeve gastrectomy, often just called the gastric sleeve) sound similar but are very different procedures. VSG is a surgical procedure: the surgeon removes about 80% of the stomach through laparoscopic incisions — permanent, requires a 1–3 day hospital stay and 2–4 weeks of recovery. ESG is non-surgical and reversible: the endoscopist places internal sutures through the mouth that fold and tighten the existing stomach — no tissue is removed, no incisions, no hospital stay, and 1–3 days of typical recovery. VSG produces greater absolute weight loss; ESG produces durable weight loss with substantially lower risk, no scars, and a much faster return to normal life.
How long does endoscopic sleeve gastroplasty (ESG) take?
The ESG stomach tightening procedure itself takes about 60 minutes. Plan on roughly half a day with us, including preparation and recovery — you'll go home the same day. No incisions, no hospital stay.
What is recovery like after ESG stomach tightening?
Most patients return to work within 1 to 3 days of their ESG stomach tightening procedure. The eating plan steps through a full-liquid phase (protein shakes), then a soft diet (oatmeal, bananas), then regular eating — typically about three weeks of progression, paced by your Everself dietitian. We provide complimentary IV hydration the day after your endoscopic sleeve gastroplasty to help you recover faster.
Is endoscopic sleeve gastroplasty reversible?
Yes. ESG stomach tightening places internal sutures that fold the stomach in on itself without removing any tissue — your physician can remove the sutures endoscopically if needed. (Note: a different procedure called endoscopic gastric plication, or EGP, folds the stomach outward and is typically not reversible. Your Everself physician will discuss which device is right for you.)
How much weight can I lose with ESG stomach tightening?
Published 5-year outcomes for endoscopic sleeve gastroplasty show an average of about 15 to 20% total body weight loss. Outcomes are strongest for patients who complete the full 12-month Everself support program — the ESG procedure tightens the stomach, but the program builds the eating patterns that hold the result long-term.
How does ESG compare to gastric sleeve surgery (VSG / sleeve gastrectomy)?
ESG stomach tightening is a non-surgical, endoscopic procedure done through the mouth — no incisions, no scars, no hospital stay, and 1 to 3 days of typical recovery. Gastric sleeve surgery (VSG, sleeve gastrectomy) removes about 80% of the stomach through laparoscopic incisions, requires a 1 to 3 day hospital stay, and 2 to 4 weeks of recovery. Sleeve gastrectomy produces greater absolute weight loss; ESG produces durable weight loss with substantially lower risk and faster return to normal life. ESG is also reversible; VSG is permanent.
How does ESG compare to gastric bypass?
Gastric bypass re-routes the small intestine through laparoscopic incisions — most aggressive procedure, longest recovery (2 to 4 weeks off work), and a 1 to 3 day hospital stay. ESG (endoscopic sleeve gastroplasty) is non-surgical, done through the mouth in about 60 minutes, with a 1 to 3 day recovery and no incisions. Bypass produces greater absolute weight loss; ESG produces durable weight loss with a fraction of the recovery and complication profile.
How does ESG compare to GLP-1 medications like Ozempic and Wegovy?
GLP-1 medications work — while you take them. Published follow-up from the STEP 1 semaglutide trial showed participants regained the majority of their weight loss within a year of stopping the medication. ESG stomach tightening is a one-time procedure paired with Everself's 12-month support program designed to build habits that hold after the procedure. For some patients, endoscopic sleeve gastroplasty can be a no-medication alternative to GLP-1s; for others, ESG can be combined with a GLP-1 to deepen results.
How much does ESG stomach tightening cost?
ESG stomach tightening with Everself costs $12,995 all-inclusive — the endoscopic sleeve gastroplasty procedure plus 12 virtual visits with your dedicated care team (physician, registered dietitian, behavioral coach). Financing is available — starting around $295/month — via Cherry and CareCredit.
Does insurance cover endoscopic sleeve gastroplasty?
Most insurance plans do not currently cover endoscopic weight-loss procedures, including ESG. Everself is a self-pay program; we keep pricing transparent and offer financing through Cherry and CareCredit so you can spread the cost of your ESG stomach tightening over 12 to 60 months.
Am I a candidate for ESG?
Endoscopic sleeve gastroplasty is generally indicated for adults with a BMI of about 30 or higher who haven't achieved durable weight loss through diet, exercise, or medication. Your Everself physician will confirm eligibility at consultation — most patients are candidates, including those who've tried GLP-1s, those who've regained weight after bariatric surgery, and those who aren't candidates for or don't want surgical sleeve gastrectomy (VSG) or gastric bypass.
How long do ESG results last? Is ESG stomach tightening permanent?
Published 5-year outcomes from Cornell (Sharaiha et al., Clinical Gastroenterology and Hepatology, 2021) show ESG stomach tightening patients maintained an average of about 15% total body weight loss at 5 years — comparable durability to laparoscopic sleeve gastrectomy (VSG) at the same follow-up. The procedure itself is durable; the 12-month Everself support program is what keeps results holding for years after.
How is Everself's ESG stomach tightening different from ESG at other providers?
Our team invented endoscopic sleeve gastroplasty at Harvard Medical School and authored both the official ESG protocol and the field's best-practice technique standard. Technique matters: ESG outcomes vary substantially with suture count, the stitch pattern, and whether each bite captures full-thickness tissue. Everself physicians average 5 to 10 times the ESG case volume of typical providers, and every endoscopic sleeve gastroplasty is wrapped in a 12-month multidisciplinary support program — not the single follow-up most clinics provide.
